Subclinical Left Ventricular Dysfunction in Asymptomatic Type 1 Diabetic Children

چکیده
J Cardiol Curr Res 2016, 6(6): 00229 measurement of left ventricular ejection fraction (LVEF) using standard two-dimensional (2D) echocardiography [2,3]. However, the LV contractility is a complex mechanism resulting from a three-dimensional structure. Myocardial fibers are orientated in different directions and responsible for three principal types of deformation: longitudinal; radial and circumferential deformation [4]. Myocardial strain assessment based on 2D speckle tracking analysis is a novel echocardiographic approach for a sensitive and angle-independent evaluation of myocardial deformation and an able tool to evaluate the three components of myocardial deformation. It has been shown to be the most sensitive echocardiographic tool for the detection of sub-clinical impairment of myocardial function observed in many conditions such as aortic stenosis, hypertrophic myocardiopathy and hypertension. The objective of this study was to characterize global systolic function abnormalities using 2D Strain in T1D children and to assess the relationship between these myocardial data and glycemic control.

Detection and potential mechanisms of subclinical left ventricular dysfunction in asymptomatic young adults with Type-2 Diabetes
Background There is an epidemic of obesity and Type-2 Diabetes Mellitus (T2DM) in the developed world. Subclinical diastolic dysfunction in diabetes is associated with the development of overt heart failure. Although diabetic cardiomyopathy is well documented in older adults with T2DM there are scarce data on younger adults and no published CMR data. We aimed to assess the prevalence and potent...
متن کاملSubclinical left ventricular dysfunction and coronary atherosclerosis in asymptomatic patients with type 2 diabetes.
AIMS The aim of the present study was to evaluate whether subclinical left ventricular (LV) systolic dysfunction is independently related to subclinical coronary atherosclerosis in type 2 diabetic patients and if it could provide incremental information over baseline characteristics to identify high-risk patients. METHODS AND RESULTS A total of 234 asymptomatic, type 2 diabetic patients witho...
متن کاملProgeria of the Heart in Type 1 Diabetic Children?
Diabetes mellitus (DM) itself may induce subclinical myo-cardial dysfunction without significant coronary artery disease. Accordingly, adults with type 2 DM are susceptible for heart failure and a lot of experimental studies have shown significant changes in microvasculature and myocardial interstitial fibro-sis in type 2 DM. 2-4) Therefore, early recognition of subclinical myocardial dysfuncti...
متن کاملEvidence for a Specific Diabetic Cardiomyopathy: An Observational Retrospective Echocardiographic Study in 656 Asymptomatic Type 2 Diabetic Patients
Aim. Our aim was to assess the prevalence of subclinical diabetic cardiomyopathy, occurring among diabetic patients without hypertension or coronary artery disease (CAD). Methods. 656 asymptomatic patients with type 2 diabetes for 14 ± 8 years (359 men, 59.7 ± 8.7 years old, HbA1c 8.7 ± 2.1%) and at least one cardiovascular risk factor had a cardiac echography at rest, a stress cardiac scintigr...
متن کاملLeft ventricular systolic function in selected type 1 diabetic patients with or without diabetic retinopathy and microalbuminuria.
Vascular endothelial dysfunction is a basic etiologic factor for the development of late clinical complications in patients with diabetes mellitus type 1, such as diabetic retinopathy, diabetic nephropathy (which is characterized at the very beginning by microalbuminuria), and left ventricular cardiac dysfunction. The aims of this study were to determine the prevalence of asymptomatic left vent...
